 CoventryOne $20 Copay Plans

Look at the CoventryOne $20 copay plans if you want a very comprehensive policy with great benefits.

Highlights of the CoventryOne $20 copay plans include:

  • $20 copay for office visits in network with a Primary Care Physician (Internist, Family Practice, Pediatrician).
  • $55 copay for office visits in network with a Specialist (OBGYN, Cardiologist, etc).
  • No limit on the number of office visits covered in network with a copay.
  • Office Visit copays include X-Ray and Lab when performed in the office and billed by the physician.
  • $20 copay for preventive screening in network - PCP and Specialist.
  • $20 copay for Convenience Care Clinics including certain Minute Clinics and Take Care Health clinics.
  • 100% coverage for mammograms in network (deductible waived).

CoventryOne offers the $20 copay plan with deductible options of $500, $1,000, $2,000, $3,000, $4,000, $5,000 and $10,000. The most popular plan is the $3,000 deductible plan. The deductible does not apply when there is a copay for a service. The deductible does apply
for services such as Inpatient Hospital, Outpatient Hospital services such as MRI, CAT & PET Scans, Surgery and Anesthesia.

CoventryOne $20 Plans include a Dental plan at no additional cost through Deltacare providers. Dental benefits include a $20 copay for
Preventive Cleanings. Various other
dental copays apply for dental services. Eye exams are covered with a $15 copay when received
from an
Avesis provider.

Applications with Coventry will be underwritten for approval, and any preexisting condition listed on the application will be covered
when the policy is approved! Coventry underwriting may either increase your premium or exclude coverage for a preexisting condition.

    Pulmonary rehabilitation.
  • Office Visit copays include X-ray and Lab when performed in office and billed by the physician.
  • $30 copay for preventive care screenings in network - PCP and Specialist.
  • $30 copay for Convenience Care clinics including certain Minute Clinics and Take Care Health Clinics.
  • 100% coverage for mammograms in network (deductible waived).
  • $250 copay for Emergency Room Services.

  Aetna Managed Choice Open Access & PPO Plans 

Aetna offers the Managed Choice Open Access plans for Georgia residents who live in Atlanta and neighboring counties, and PPO
plans for all other Georgia counties. The benefits on the Managed Choice Open Access plans are identical to the Georgia PPO plans,
only the networks are different. Aetna uses the Managed Choice Open Access (Aexcel) network for the Managed Choice Open Access plans and the Aetna Advantage PPO network for the Aetna PPO plans. 

Highlights of the Aetna Managed Choice Open Access and PPO plans:

  • Deductible options of $1,500, $2,500, $3,500 and $5,000.
  • Unlimited office visit copayments to either a Primary physician or Specialist.
  • Office visit copayments from $25 to $50 depending on the deductible chosen.
  • $50 Urgent Care Facilty copay.
  • $350 Emergency Room copay.
  • 100% coverage with no deductible in network for Annual Routine GYN exam including Pap and mammogram.
  • Office visit copay applies for Preventive Health Routine Physical in network up to $250 per exam.
  • $15 copayment for generic prescriptions with no Rx deductible.
  • Optional dental insurance 

Applications with Aetna will be underwritten for approval, and any preexisting condition listed on the application will be covered under the policy as long as you have had previous coverage in effect with no longer than a 63 day lapse. Aetna underwriting may increase your premium due to preexisting health conditions, but they will not specifically exclude coverage for a preexisting condition. Please contact

    HumanaOne Enhanced Copay 80% plan

Humana offers their most benefit rich policy with the Enhanced Copay 80% plan.

Highlights of the Enhanced Copay 80% plan:

  • Deductible options of $500, $1,000, $1,500, $2,000, $2,500, $3,500 and $5,000.
  • Unlimited office visits to a Primary Physician with a $35 copay.
  • Unlimited office visits to a Specialist with a $60 copay.
  • Lab and X-rays covered at 100% up to $500 per person per calendar year, then you pay 20% after your deductible. Includes
    Pap smear, prostate screening, mammogram and allergy testing.
  • Plan pays 80% after your deductible for services without a copay, such as Inpatient hospital services.
  • $15 copay for generic prescriptions with no deductible.
  • Deductible carryover credit - any portion of your deductible met in the last three months of the calendar year apply towards your medical
    deductible for the next year.

Optional benefits with Humana's Enhanced Copay 80% plan:

  • A Traditional dental plan.
  • Individual Life Insurance.
  • A Supplemental accident benefit that pays a set amount per covered person for treatment of an accident at 100%. Choose between a
    Supplemental accident benefit amount of either $1,000 or $2,500.
  • Enhanced Mental health benefit - this extra benefit includes coverage for Chemical and Alcohol dependence the same as any other illness.

   Blue Cross Blue Shield's Premier Plans

Blue Cross offers the benefit rich Premier plans.

Premier Plan Features:

  • The Premier plans offer individual deductibles of $750, $1,500, $2,500, $5,000, $10,000 and $20,000. The family maximum
    deductible is two times the individual deductible.
  • $35 office visit copayments for physician office visits (Primary or Specialist) and preventive care.
  • Preventive Care Testing including Immunizations, Flu Injections, Mammography and Pap smear paid at 80% not subject
    to the deductible.
  • Generic prescriptions are covered at the greater of either $15 or 40% of the prescription cost.
  • Brand Name prescriptions are covered subject to a seperate $250 Rx deductible, then a $15 copay or 40%
    (whichever is greater) plus the difference in allowable charge if Brand is chosen over an available generic. 
     
  • All other covered serices paid at 80% after the deductible up to an individual out of pocket maximum of $2,500 plus
    your deductible. Lifetime maximum is 7 million per member. Plan deductibles of $10,000 and $20,000 pay 100%
    after the deductible.

Maternity is an optional benefit with the Premier Plans. Maternity physician care is subject to the deductible and coinsurance, 
while Hospital Facility charges are subject to a $3,000 copayment. The extra premium for maternity benefits is $152.77 per month.

Applications with Blue Cross for the Premier Plans will be medically underwritten and some preexisting health conditions may
increase you're monthly premium. Please contact us for a prescreen if you're concerned about a preexisting condition.
