Coventry One $20 copay Plans
Coventry has been popular in Georgia for years in the small group market and they now offer very competetive plans for individuals and families.
Highlights of the Coventry One copay plans include:
- $20 copay for office visits in network with a Primary Care Physician (Internist, Family Practice, Pediatrician).
- $55 copay for office visits in network with a Specialist (OBGYN, Cardiologist, etc).
- No limit on the number of office visits covered in network with a copay.
- Office Visit copays include X-Ray and Lab when performed in the office and billed by the physician.
- $20 copay for preventive screening in network - PCP and Specialist.
- 100% coverage for mammograms in network.
- $150 copay for Emergency Room Services.
- $55 copay for Urgent Care Centers.
- $10 copay for Chiropractic Services in network up to 24 visits.
- $10 copay for generic presriptions with no deductible.
Coventry One offers plan Deductible options of $500, $1,000, $2,000, $3,000, $4,000, $5,000 and $10,000. The most popular plan is the $3,000 deductible plan. The deductible does not apply when there is a copay for a service. The deductible does apply for services such as Inpatient Hospital, Outpatient Hospital services such as MRI, CAT & PET Scans, Surgery and Anesthesia.
Coventry One Plans include a Dental plan at no additional cost through Deltacare. Dental benefits include a $20 copay for Preventive Cleanings. Various other copays apply for dental services. Eye exams are covered with a $15 copay when received from an Avesis provider.
Applications with Coventry will be underwritten for approval, and any preexisting condition listed on the application will be covered when the policy is approved!

Aetna Managed Choice Open Access & PPO Plans
Aetna offers the Managed Choice Open Access plans for Georgia residents who live in Atlanta and neighboring counties, and PPO plans for all other Georgia counties. The benefits on the Managed Choice Open Access plans are identical to the Georgia PPO plans, only the networks are different. Aetna uses the Managed Choice Open Access (Aexcel) network for the Managed Choice Open Access plans and the Aetna Advantage PPO network for the Aetna PPO plans.
Highlights of the Aetna Managed Choice and PPO plans:

Two "No Deductible Plans" (First Dollar 30 and First Dollar 40). The First Dollar 30 plan has a copayment of $30 for Primary Care office visits and a copayment of $40 for Specialist office visits. The First Dollar 40 plan has a copayment of $40 for Primary Care office visits and a copayment of $50 for Specialist office visits.

Four Deductible plans - $1,500, $2,500, $3,500 and $5,000. These plans offer Primary Care office visit copayments of $25-$40 and Specialist office visit copayments of $35-$50 depending upon the deductible selected. Aetna plans pay 80% after deductible up to out of pocket maximum.

Aetna's plans are unique in that the underwriters do not apply exclusion riders for pre-existing conditions, and also offer coverage for pre-existing conditions if you provide proof of prior coverage with no lapse greater than 63 days. Humana One Health Plans
Humana offers two popular PPO plans:
Portrait Plan
Humana's Portrait plan is a PPO plan that pays 80% after either a $1,000 or $2,500 deductible.
Plan features include:

Preventive Care including routine physical exam covered at 100% with no deductible ($300 per year).

Family Deductibles and Maximum out of pocket expenses are limited to two times the single deductible. For instance, with the $1,000 deductible plan the family maximum deductible is $2,000, where as most other carriers would have the maximum family deductible as three times the single deductible or $3,000.

Autograph Share 80 Plus Rx
Humana's Autograph Share 80 Plus Rx plan is a PPO plan that pays 80% after either a $5,000 or $6,000 deductible.

Blue Cross Blue Shield's SmartSense & Premier Plans
Blue Cross offers a cost savings PPO plan with the SmartSense plans and more benefit rich Premier plans. Both plans use the individual Blue Cross PPO network - one of the largest in the state with direct access to any PPO provider.
The Smart Sense plans offer individual deductibles of $750, $1,500, $2,500, $5,000, $10,000 and $20,000. The family maximum deductible is two times the individual deductible.

$30 office visit copayment for the first three visits per member, per year, not subject to the deductible including preventive care visits. After three visits physician office charges are discounted in network, then paid at 70% after the deductible. Preventive Care visits for Children through age 5 after three visits are covered at 70% not subject to the deductible.

Generic prescriptions are covered at the greater of either $15 or 40% of the prescription cost.

Brand Name prescriptions are covered (with the optional comprehensive drug benefit) subject to a seperate $250 Rx deductible, then a $15 copay or 40% (whichever is greater) plus the difference in allowable charge if Brand is chosen over an available generic.

- Generic prescriptions are covered at the greater of either $15 or 40% of the prescription cost.
- Brand Name prescriptions are covered subject to a seperate $250 Rx deductible, then a $15 copay or 40%
(whichever is greater) plus the difference in allowable charge if Brand is chosen over an available generic.

Maternity is an optional benefit with the $750, $1,500, $2,500 and $5,000 deductible Premier Plans. Maternity physician care is paid at 80% after the deductible, while Hospital Facility charges are subject to a $3,000 copayment. The extra premium for maternity benefits is $144 per month.
